Team Manager and Assistant must complete an Administrator Registration Form

Player fee SNE/OSA/CSA/book $26.00 unused will be refunded minimum 16 player registered.
Performance bond will be deducted $100.00 per default and must be replaced to continue play.
No defaults will be complete refund of bond.
Minimum $500.00 to be submitted by April 15/11, 50 % Fees by April 30 and Balance May15/11

Player registration form must be completed and approved by the team manager. Please be aware we are no longer using player books for personal identification. The option to generate player books for any team is the team responsibility. Any team participating in any exhibition game or tournament requires district validated player books.
For new registrants a copy of some proof of id must accompany registration.
This id will be destroyed after data is entered to the OSA player registration system.
Address, phone and email should be completed. Email is our primary method of contact.

Completion online is available and preferred for readability. Team manager should receive and approve all player forms and submit all required documents to the Club Registrar.
